The following section will show you all information related to the EZ Attestation forms

Here below there’s the list of attributes you will get access to:

  • Date sent

  • Sent by

  • Patient

  • Pharmacy

  • Sent to

  • Delivery Status

  • Delivery Error

  • Attestation Form Viewed and Signed status.

 

Here below there’s the list of actions you will get access to:

  • Attestation View: Will be able to see the actual AF.

  • Re-Send the AF.

Also, for an easier or more specific search, there are some filters available in the upper-right corner where the user can enter the Patient First Name or Last Name, Phone Number or any Date Range.

By clicking on the “Statistics” button, three graphics will be displayed showing some information about AF sent in the last 60 minutes, last 24 hours and last 7 days.
